How to reduce no-shows in a therapy practice

By Nerela · Published 23 September 2026 · 7 min read

Every no-show is a slot that doesn't come back and income that's gone — and for the patient, a break in continuity of care that can matter clinically as much as financially. The scale of the problem in Ireland's public system is well documented: over half a million outpatient hospital appointments went unattended in 2024 alone. In private practice the fix is smaller in scope but just as real. Here's what the evidence actually says works.

What no-shows really cost

Picture a practice running 20 sessions a week at €70, with a 12% no-show rate. That's roughly €645 a month lost to empty chairs — close to €7,700 a year — before you even count the admin time spent chasing rebookings. Seeing the number in euro is usually what turns "I should sort this out" into actually doing it.

Want your own number? Our free cost of missed appointments calculator shows exactly what no-shows are costing your practice, in about two minutes.

1. Automatic reminders

The single highest-return change most practices can make. An Irish Department of Health trial found that a behaviourally-informed SMS reminder reduced hospital outpatient no-shows (DNAs) by 13%, and a controlled trial in mental health settings (Kaiser Permanente) found an 11% drop from one additional text message, with combined email, SMS and WhatsApp reminders reported to cut no-shows by up to 50% in other health-service studies. Forgetting, not reluctance, is the most common reason people miss an appointment — a well-timed reminder addresses that directly, and it works best when it happens on its own, without you having to remember to send it.

2. A clear cancellation policy

Set out, in writing, at the first session, what happens if a patient cancels late or doesn't show up. A fair, transparent policy — for example, asking for 24 hours' notice — puts the responsibility where it belongs without damaging the therapeutic relationship, as long as it's applied consistently rather than decided case by case.

3. Make it easy to cancel or reschedule

It sounds counterintuitive, but making cancellation easy reduces no-shows. A patient who can move their appointment in one click frees the slot early enough for someone else to take it. What you want to avoid is silence on the day of the session, with no notice at all.

4. A waitlist to fill gaps

When a slot frees up, an organised waitlist lets you offer it to another patient straight away. A late cancellation stops being a pure loss and becomes a chance for someone else to be seen sooner.

5. Active confirmation for higher-risk appointments

For first sessions, or patients with a history of missed appointments, an active confirmation — where the patient replies to confirm, rather than just receiving a passive reminder — increases commitment. Many systems let you automate this request rather than chasing it by hand.

Bringing it together

Each of these measures helps on its own. Combined and automated, they change the shape of your diary. Practice management software that sends reminders, allows online booking and cancellation, and keeps a waitlist does this work for you — without you having to think about it every week. Frequently asked questions

What's a normal no-show rate for a therapy practice?+

It varies a lot, but rates of 10-20% are common in private practice without automatic reminders. In the Irish public hospital system, outpatient DNA rates have been reported around 13-18%, and higher again for some specialties. With reminders and a clear policy in place, many practices see that fall by roughly half.

Do reminder texts and emails actually work?+

Yes, and the evidence is specific: an Irish Department of Health trial found a behaviourally-informed SMS reminder cut hospital outpatient no-shows by 13%, and a controlled trial in mental health settings found an 11% drop from a single extra text. Forgetting is the single biggest cause of no-shows, and a well-timed reminder is the highest-return fix available.

Should I charge for missed sessions?+

That's a decision for each practitioner, and views differ across the profession. What matters most is having a clear policy, put in writing and explained at the first session, and applying it consistently — rather than deciding case by case after the fact.
